Progesterone, along with pregnenolone, is the biosynthetic precursor of all other steroid hormones. Progesterone is synthesized from cholesterol by the sequential action of desmolase in the mitochondria, which produces pregnenolone, followed by ?4,5-isomerase in the outer mitochondrial membrane and smooth endoplasmic reticulum of steroid-secreting cells.{8458} The main function of progesterone is to prepare the uterine lining for implantation of a fertilized ovum and to maintain pregnancy. Measurement of serum or plasma progesterone levels are used as an index to monitor ovulation and investigate luteal function. Plasma concentrations of progesterone are approximately 0.2-0.8 ng/ml and 4-20 ng/ml during the follicular phase and luteal phase of the menstrual cycle, respectively.{8458} Salivary concentrations of progesterone are approximately 100-fold lower than those found in plasma.{1639}
